BAB IV -\u00a0TATA LAKSANA PELAYANAN. \u00a0 4.1.PENDAFTARAN DAN PENCATATAN. Pasien yang datang kelaboratorium terdiri dari beberapa jenis berdasarkan permintaan pemeriksaan laboratorium , sebagai berikut : Pasien dari dokter rawat jalan RS Pasien dari dokter IGD RS Pasien dari dokter rawat inap RS Pasien dari dokter luar rumah sakit ( dokter Askes dan Non Askes ) Pasien Medical Check-Up (MCU) Pendaftaran untuk pasien Rumah Sakit dilakukan di bagian masing-masing asal pasien, tapi formulir permintaan dari bagian asal pasien disimpan dilaboratorium sebagai arsip dan hasil laboratorium dicatat dalam bentuk softcopy ( program computer ) maupun hardcopy di buku pencatatan hasil kegiatan laboratorium. Pendaftaran untuk pasien yang membawa formulir permintaan\u00a0 pemeriksaan laboratorium dari dokter luar , langsung dilakukan di laboratorium dan hasil laboratorium dicatat dalam bentuk softcopy ( program computer ) maupun hardcopy di buku pencatatan hasil kegiatan laboratorium. Untuk pasien-pasien MCU, pendaftaran dilakukan melalui Tim MCU, arsip permintaan dan hasil laboraotium akan dicatat dan disimpan di laboratorium bentuk softcopy ( program computer ) maupun hardcopy di buku pencatatan hasil kegiatan laboratorium. 4.2.\u00a0 PENGELOLAAN SPESIMEN. Spesimen infeksius Spesimen tidak infeksi Semua bahan pemeriksaan\/sampel dianggap infeksius karena berasal dari manusia yang tidak diketahui apakah infeksius atau tidak.Semua bahan pemeriksaan (sampel) diberikan identitas pasien. Identitas pasien ini menurut nama, alamat, RM,umur. Pada surat permintaan pemeriksaan laborat juga ditulis : Tanggal permintaan Jam pengambilan sampel Identitas pasien yang meliputi nama, umur, jenis kelamin, alamat,RM, ruangan tempat Identitas dokter Diagnosis dan keterangan Pemeriksaan laboratorium yang Nama pengambil Pengelolaan Serum \u00a0 Darah dibiarkan membeku terlebih dahulu pada suhu kamar selama 10 \u2013 20 menit, kemudian dicentrifuge pada 3000rpm selam 5 \u2013 15 Serum dipisahkan dari komponen darah yang lain. Serum yang memenuhi syarat apabila tidak lisis (kelihatan merah) atau keruh (lipemik). Pengelolaan darah EDTA \u00a0 Sampel darah yang telah diambil dan mengalir ke dalam tabung yang telah berisi EDTA dicampurkan dengan cara membolak-balikkan tabung dengan hati-hati sebanyak 8 Pada saat hendak dilakukan pemeriksaan dilakukan pencampuran kembali dengan cara dibolak-balikkan sebanyak 5 \u2013 10 Pengelolaan Plasma \u00a0 Sampel darah yang telah diambil dan mengalir ke dalam tabung yang telah berisi natrium citrate 0,38% dicampurkan dengan cara membolak-balikkan tabung dengan hati-hati sebanyak 4 Plasma dipisahkan tidak boleh lebih dari 2 jam setelah pengambilan sampel. Plasma yang memenuhi syarat tidak kelihatan merah (lisis) atau keruh ( lipemik). Pengelolaan Urine \u00a0 Untuk uji carik celup, urine tidak memerlukan perlakuan khusus, kecuali pemeriksaan harus segera dilakukan dalam waktu 1 jam, sedangkan untuk pemeriksaan sedimen harus dilakukan pengeloaan terlebih dahulu dengan cara : Urine sebanyak \u00b1 10 ml dimasukkan ke dalam tabung Urine dicentrifuge selama 5 menit 3000rpm Supernatan urine dibuang dengan menyisakan 1 ml, kemudiaan dikocok untuk meresuspensikan Serum yang akan dikirim ke laboratorium lain sebaiknya dalam bentuk yang relative stabil. Untuk itu persyaratan yang dipenuhi untuk pengiriman sampel : Stabilitas serum tidak melampaui batas waktu yang Tidak terkena sinar matahari Dimasukkan ke dalam sumple cup yang tertutup Untuk pemeriksaan mikrobiologi menggunakan media 4.3.\u00a0 PEMERIKSAAN LABORATORIUM. Hematologi Laboratorium Klinik RS\u00a0\u00a0 memiliki 2 alat hematologi analiser yang berfungsi sebagai alat utama dan alat \u201cbackup\u201d. Alat utama adalah analiser dengan kemampuan 5 differensiasi sel lekosit sedangkan alat \u201cbackup\u201d memiliki kemampuan 3 differensiasi sel lekosit. Pemeriksaan laboratorium lain yang tercakup dalam hematologi adalah LED, pemeriksaan hapusan darah\/ morfologi darah, pemeriksaan golongan darah. Faal Hemostasis Faal hemostasis adalah pemeriksaan untuk fungsi koagulasi darah. Yang dapat dilakukan di laborarotium adalah pemeriksaan Plasma Protrombin Time ( PPT ) dan Aktivated Partial Thromboplastin Time ( APTT ), Cloting Time , Bleeding Time dan Retraksi bekuan. Untuk pemeriksaan faktor-faktor pembekuan\u00a0 akan di rujuk ke laboratorium rujukan. Kimia Klinik Parameter Kimia klinik dilaboratorium RS\u00a0\u00a0 yang tersedia adalah parameter untuk pemeriksaan fungsi hati, fungsi ginjal, profil lemak, petanda penyakit jantung\/cardiovaskuler, diabetes dan monitoringnya. Elektrolit dan Analisa Gas Darah Pemeriksaan elektrolit menggunakan metode ISE untuk Natrium, Kalium, Chlorida dan Kalsium. Sementara Fosfor dan Magnesium digunakan metode Kimiawi. Untuk pemeriksaan Anlisa Gas Darah dipakai catridge ( reagen kering ). Urinalisa Urinalisa memakai 10 parameter dalam 1 strip urin.\u00a0 Metode semiautomatic, karena tes kimiawi dibaca hasilnya dengan alat sedangkan mikroskopis sel masih manual , yaitu pembacaan dengan mikroskop. Beberapa tes imunoserologi yang ada merupakan rapid tes, dan sebagian merupakan tes aglutinasi manual . Tidak ada autoanalyzer untuk pemeriksaan imunoserologi. Mikrobiologi dan Parameter mikrobiologi yang dilakukan hanya pengecatan Gram dan Ziel Nielsen. Patologi Anatomi ( PA ). Untuk pemeriksaan Pattologi anatomi laboratorium RS\u00a0\u00a0 merujuk ke laboratorium PA\u00a0\u00a0 yang bekerjasama dengan Rumah Sakit. Untuk pemeriksaan CITO \/ IGD hasil dapat diketahui : 15 menit untuk pemeriksaan darah lengkap tanpa ada 1 Jam untuk pemeriksaan kimia Untuk pemeriksaan regular \/ biasa dapat diketahui : Pasien Rawat Jalan : Semua pemeriksaan dilakukan tidak lebih dari 2 jam ( standart waktu tunggu ? 120 menit). Pasien Rawat Inap : Dalam waktu 2 jam setelah pengambilan sampel untuk semua jenis pemeriksaan. Untuk pemeriksaan rujukan hasil dapat diketahui : -\u00a0\u00a0\u00a0\u00a0 Dalam waktu 1 minggu setelah pengiriman sampel. Bila ditemukan hasil yang kritis dari pemeriksaan diagnostic dilaporkan dengan system pelaporan yang formal (yang benar). Laboratorium rumah sakit menetapkan nilai ambang kritis bagi setiap jenis pemeriksaan, bila ada hasil yang kritis segera dilaporkan oleh petugas laborat ke perawat ruangan \/ dokter yang meminta pemeriksaan melalui via telepon, sebelum hasil dikeluarkan dan didokumentasikan di buku laporan. Hasil pemeriksaan dicatat di blanko permintaan pemeriksan Hasil pemeriksaan tersebut diketik di computer rangkap dua untuk pasien umum, copi pertama untuk arsip di rekam medis dan ditempel, copi kedua diberikan kepada Rangkap tiga untuk pasien askes dan asuransi, copi pertama untuk arsip di rekam medis, copi kedua untuk klaim asuransi \/ askes, copi ketiga untuk diberikan kepada pasien. Hasil pemeriksaan untuk pasien rawat inap diantar oleh petugas laboratorium ke\u00a0\u00a0\u00a0 masing-masing\u00a0\u00a0\u00a0 ruangan,\u00a0\u00a0\u00a0 dengan\u00a0\u00a0\u00a0 membawa buku ekspedisis\u00a0\u00a0\u00a0\u00a0 laboratorium\u00a0\u00a0\u00a0\u00a0 ,\u00a0\u00a0\u00a0\u00a0 untuk\u00a0\u00a0\u00a0\u00a0 ditandatangani\u00a0\u00a0\u00a0 perawat\u00a0\u00a0\u00a0\u00a0 yang bersangkutan. 4.4\u00a0\u00a0 Daftar Nilai Normal dan Nilai Kritis \u00a0 Nilai Normal No Tes Nilai Normal Pria Nilai Normal Wanita Satuan 1. PARAMETER YANG DI RUJUK. \u00a0 Beberapa parameter yang tidak dapat dikerjakan dilaboratorium akan di rujuk ke laboratorium rujukan yaitu Prodia Malang ,Kedungdoro Surabaya dan Lab RS\u00a0\u00a0 Kediri. Parameter yang cukup banyak dirujuk akan diusahakan untuk diadakan . Keputusan pengadaan parameter baru berdasarkan evaluasi, efisiensi dan efektifitas. 4.6.\u00a0\u00a0 PENGELOLAAN LIMBAH \u00a0 Sampah Medis berupa peralatan flebotomi, tabung specimen akan dibuang di tempat sampah dengan plastiK kuning dan diteruskan masuk ke insenerator Sampah Medis berupa sisa specimen \/ cairan tubuh akan dibuang ke bak pencucian dimana salurannya akan diteruskan ke bak pengolahan limbah cair Sampah bukan medis , seperti kertas, tissue, plastic dan lain-lain akan dibuang ke tempat sampah dengan plastic hitam dan akan dibuang ke tempat pembuangan sampah akhir 4.7.\u00a0\u00a0 LAPORAN HASIL DAN ARSIP. \u00a0 Laporan hasil laboratorium akan dibuat arsip dalam bentuk hardcopy dan softcopy. Rekapitulasi hasil laboratorium juga dibuat dalam bentuk Buku Laporan Kinerja Laboratorium dan dalam bentuk Laporan Bulanan. 4.8.\u00a0\u00a0 PEMELIHARAAN DAN KALIBRASI ALAT. \u00a0 Pemeliharaan alat-alat laboratorium dilakukan oleh prinsipal alat dengan jadwal 1x\/ bulan. Perawatan harian dan mingguan dilakukan oleh analis pekerja laboratorium. Kalibrasi alat dikerjakan sesuai dengan kebutuhan. Untuk alat elektrolit , kalibrasi otomatis dilakukan oleh alat beberapa kali sehari. Untuk alat kimia klinik kalibrasi akan dilakukan bila kontrol kualitas tidak baik ( tidak masuk dalan batas kontrol \/ tidak terkontrol ). Sementara itu beberapa mikropipet akan dikalibrasi 1 \u2013 2x \/ tahun dengan mengirimnya ke Badan Kalibrasi tersertifikasi. TROUBLE SHOOTING \u00a0 Pemecahan masalah dalam laboraotrium terutama masalah dengan pemeriksaan diatur dalam panduan trouble shooting.\", \"Indonesian Female\");\n                }\n            };\n        <\/script>\n    <\/p>\n<p><span style=\"color: #000000;\"><strong>BAB IV &#8211;\u00a0<\/strong><strong>TATA LAKSANA PELAYANAN.<\/strong><\/span><\/p>\n<p><span style=\"color: #000000;\"><strong>\u00a0<\/strong><\/span><\/p>\n<p><span style=\"color: #000000;\"><strong>4.1.PENDAFTARAN DAN PENCATATAN.<\/strong><\/span><\/p>\n<p><span style=\"color: #000000;\">Pasien yang datang kelaboratorium terdiri dari beberapa jenis berdasarkan permintaan pemeriksaan laboratorium , sebagai berikut :<\/span><\/p>\n<ol>\n<li><span style=\"color: #000000;\">Pasien dari dokter rawat jalan RS<\/span><\/li>\n<li><span style=\"color: #000000;\">Pasien dari dokter IGD RS<\/span><\/li>\n<li><span style=\"color: #000000;\">Pasien dari dokter rawat inap RS<\/span><\/li>\n<li><span style=\"color: #000000;\">Pasien dari dokter luar rumah sakit ( dokter Askes dan Non Askes )<\/span><\/li>\n<li><span style=\"color: #000000;\">Pasien <em>Medical Check-Up <\/em>(MCU)<\/span><\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<p><span style=\"color: #000000;\">Pendaftaran untuk pasien Rumah Sakit dilakukan di bagian masing-masing asal pasien, tapi formulir permintaan dari bagian asal pasien disimpan dilaboratorium sebagai arsip dan hasil laboratorium dicatat dalam bentuk <em>softcopy <\/em>( program computer ) maupun <em>hardcopy <\/em>di buku pencatatan hasil kegiatan laboratorium.<\/span><\/p>\n<p><span style=\"color: #000000;\">Pendaftaran untuk pasien yang membawa formulir permintaan\u00a0 pemeriksaan laboratorium dari dokter luar , langsung dilakukan di laboratorium dan hasil laboratorium dicatat dalam bentuk <em>softcopy <\/em>( program computer ) maupun <em>hardcopy <\/em>di buku pencatatan hasil kegiatan laboratorium.<\/span><\/p>\n<p><span style=\"color: #000000;\">Untuk pasien-pasien MCU, pendaftaran dilakukan melalui Tim MCU, arsip permintaan dan hasil laboraotium akan dicatat dan disimpan di laboratorium bentuk <em>softcopy <\/em>( program computer ) maupun <em>hardcopy <\/em>di buku pencatatan hasil kegiatan laboratorium.<\/span><\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<h1><span style=\"color: #000000;\">4.2.\u00a0 PENGELOLAAN SPESIMEN.<\/span><\/h1>\n<ol>\n<li><span style=\"color: #000000;\">Spesimen infeksius<\/span><\/li>\n<li><span style=\"color: #000000;\">Spesimen tidak infeksi<\/span><\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<p><span style=\"color: #000000;\">Semua bahan pemeriksaan\/sampel dianggap infeksius karena berasal dari manusia yang tidak diketahui apakah infeksius atau tidak.Semua bahan pemeriksaan (sampel) diberikan identitas pasien. Alat utama adalah analiser dengan kemampuan 5 differensiasi sel lekosit sedangkan alat \u201cbackup\u201d memiliki kemampuan 3 differensiasi sel lekosit. Pemeriksaan laboratorium lain yang tercakup dalam hematologi adalah LED, pemeriksaan hapusan darah\/ morfologi darah, pemeriksaan golongan darah.<\/span><\/p>\n<p>&nbsp;<\/p>\n<ol>\n<li><span style=\"color: #000000;\">Faal Hemostasis<\/span><\/li>\n<\/ol>\n<p><span style=\"color: #000000;\">Faal hemostasis adalah pemeriksaan untuk fungsi koagulasi darah. Yang dapat dilakukan di laborarotium adalah pemeriksaan Plasma Protrombin Time ( PPT ) dan Aktivated Partial Thromboplastin Time ( APTT ), Cloting Time , Bleeding Time dan Retraksi bekuan. Untuk pemeriksaan faktor-faktor pembekuan\u00a0 akan di rujuk ke laboratorium rujukan.<\/span><\/p>\n<ol>\n<li><span style=\"color: #000000;\">Kimia Klinik<\/span><\/li>\n<\/ol>\n<p><span style=\"color: #000000;\">Parameter Kimia klinik dilaboratorium RS\u00a0\u00a0 yang tersedia adalah parameter untuk pemeriksaan fungsi hati, fungsi ginjal, profil lemak, petanda penyakit jantung\/cardiovaskuler, diabetes dan monitoringnya.<\/span><\/p>\n<ol>\n<li><span style=\"color: #000000;\">Elektrolit dan Analisa Gas Darah<\/span><\/li>\n<\/ol>\n<p><span style=\"color: #000000;\">Pemeriksaan elektrolit menggunakan metode ISE untuk Natrium, Kalium, Chlorida dan Kalsium. Sementara Fosfor dan Magnesium digunakan metode Kimiawi. Untuk pemeriksaan Anlisa Gas Darah dipakai catridge ( reagen kering ).<\/span><\/p>\n<ol>\n<li><span style=\"color: #000000;\">Urinalisa<\/span><\/li>\n<\/ol>\n<p><span style=\"color: #000000;\">Urinalisa memakai 10 parameter dalam 1 strip urin.\u00a0 Metode semiautomatic, karena tes kimiawi dibaca hasilnya dengan alat sedangkan mikroskopis sel masih manual , yaitu pembacaan dengan mikroskop.<\/span><\/p>\n<p><span style=\"color: #000000;\">Beberapa tes imunoserologi yang ada merupakan rapid tes, dan sebagian merupakan tes aglutinasi manual . Tidak ada autoanalyzer untuk pemeriksaan imunoserologi.<\/span><\/p>\n<ol>\n<li><span style=\"color: #000000;\">Mikrobiologi dan<\/span><\/li>\n<\/ol>\n<p><span style=\"color: #000000;\">Parameter mikrobiologi yang dilakukan hanya pengecatan Gram dan Ziel Nielsen.<\/span><\/p>\n<ol>\n<li><span style=\"color: #000000;\">Patologi Anatomi ( PA ).<\/span><\/li>\n<\/ol>\n<p><span style=\"color: #000000;\">Untuk pemeriksaan Pattologi anatomi laboratorium RS\u00a0\u00a0 merujuk ke laboratorium PA\u00a0\u00a0 yang bekerjasama dengan Rumah Sakit.<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"color: #000000;\"><u>Untuk pemeriksaan CITO \/ IGD hasil dapat diketahui :<\/u><\/span><\/p>\n<p>&nbsp;<\/p>\n<ol>\n<li><span style=\"color: #000000;\">15 menit untuk pemeriksaan darah lengkap tanpa ada<\/span><\/li>\n<li><span style=\"color: #000000;\">1 Jam untuk pemeriksaan kimia<\/span><\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<p><span style=\"color: #000000;\"><u>Untuk pemeriksaan regular \/
